• Home infusion registered nurse – familiar with all types of peripheral and central lines used for intravenous infusion. This nurse performs a comprehensive assessment with each nursing visit and will train the patient or a caregiver how to properly administer medications at home. A primary RN case manager follows the patient’s progress and develops a patient-specific plan of care based on patient needs and physician orders. The nurse monitors the patient’s progress throughout the therapy and will report progress or complications to the ordering physician.
• Home infusion pharmacist – completes a pharmacy assessment including medical history review, medication allergy history, medication profile review and review of any medication-related problems. Via telephone and through the nursing staff, the pharmacist will assess, on an ongoing basis, side effects and medication problems and outcomes.
• Home infusion account specialist – an expert who will review a patient’s insurance benefits prior to infusion therapy. The patient and caregivers are informed of coverage and any financial responsibility.
• Home infusion delivery technician – delivers all necessary supplies for infusion to the patient’s home.
