New Hire Requirements


We require direct deposit for all associates' paychecks.  You can use any bank, checking or saving account, or split funds between two accounts.  We have a credit union if you would like to join.  We will ask you to bring a deposit slip or a voided check with you at the time you complete your new hire paperwork in Human Resources.


All associates must attend General Orientation, which is held twice a month on a Monday and Tuesday, from 8 a.m.-4:30 p.m.  It is required that all new hires attend both days of general orientation.  You will not be permitted to start in the department until both days of general orientation are completed.


The first 90 days of employment is considered the "Introductory Period."  During this time period all associates are reviewed at 45 days and 90 days to ensure that the introductory period is proceeding satisfactorily.  Half-time and full-time associates do not earn PAM time during this period.


We require fingerprinting and background checks for all of our positions.   If you are offered a position, we will ask that you provide "proof of residency" for the state of Ohio at the time you complete your new hire paperwork.  We will ask you to provide residency documentation for the last five years (one document for each year for the last five years, to total five separate pieces of paper) showing you lived here in the state of Ohio.  This can be done with tax records, utility bills, report cards, etc.  If you have not lived in the state of Ohio for the last five years, we will do a more extensive, nationwide fingerprinting check.


We require a pre-placement physical and urine drug screen test with a SWGHC Urgicare within 48 hours of accepting an offer of employment. 
 
Pre-Placement Physical includes:
Two-step TB test, external exam and blood draw

You will be requested to bring with you:
Rubella titer, rubeola titer, varicella Titer, tetanus date, hepatitis vaccine or titer, TB test, and a photo ID
