Thank you for your interest in receiving care from Southwest General Health Center.
As of October 28, 2022, Southwest General is no longer offering On-Demand Care.
Instead, to help meet patient needs, our
Southwest General Medical Group (SGMG) providers are welcoming both current and new patients and offer several convenient
appointment options.
SGMG appointment options include:

In-person Visits
An in-person office visit may be scheduled for any non-emergency care that
requires in-person assessment, treatment or procedures.
Both current and new patients are invited to schedule an in-person appointment
in one of the following ways:

Call the provider’s office directly.

- Use your Southwest General HealtheLife (patient portal) account to send
the provider a message requesting an appointment. Someone from the office
will then contact you about your appointment.

Scheduled Telehealth/Virtual Visits
If you do not need in-person care and prefer to visit with your provider
from the convenience of home, you can schedule a telehealth (virtual)
visit. Your provider's office will help determine if in person care is
required or if a telehealth visit is appropriate.
Telehealth visits may be scheduled by current SGMG patients only. For new
patients, an in-person visit is required for the first appointment, after
which telehealth visits may be scheduled as needed.
