Southwest General provides patients with access to their medical records in various ways. Additionally, patients and their Personal Representatives have the right to review and request amendments to their medical information. For more information regarding patients’ rights under the HIPAA laws, refer to our Notice of Privacy Practices webpage.

The HealtheLife patient portal provides individuals with secure online access to portions of the electronic medical record 24/7/365. Creating an account is free and takes only a few minutes.

Electronic Request (eRequest) for Records

All Southwest General records, including hospital records and SGMG Physician Office records, can now be easily requested using the link below, which goes to a secure external site hosted by Datavant, our release of information partner. The links below can be used to request records for their designated purposes. Identity will be verified at the time of request.

Paper Request for Records

You may print and submit your request for hospital records or SGMG Physician Office records in writing directly to the Medical Records Department. The form may be emailed to swroi@swgeneral.com or mailed/dropped off at the address at the bottom of the form. For questions, please call Medical Records at 440-816-8480.

Request an Amendment to Medical Records

Patients have a right to request changes, or an amendment, to their medical record when they believe the information may be inaccurate as written. For convenience, the following form may be used to submit a request to amend the medical record. Southwest is not obligated to make all requested changes however careful consideration will given to each request. All requests must be in writing (signed and dated).
