The Thomas P. Perciak Family Residential Hospiceof Southwest General offers a comfortable setting staffed by professional hospice personnel ready to provide compassionate and comprehensive inpatient care in a home-like environment.

Levels of Care

Symptom Management

This level of care involves frequent nursing assessment and care planning to address symptoms that cannot be easily managed at home or in another setting. This is typically a short-term stay in our residential center with a goal of returning home or to another facility. The hospice attending physician is involved in determining this level of care’s duration.

Respite Care

Respite care services are provided at our residential center to allow the caregiver a time to rest or attend to personal matters, knowing that the patient is being cared for by trained staff members. To inquire about the use of respite care, please talk with your hospice nurse or social worker.

Residential Care

This level of care is intended for short-term use with the acknowledgement that each patient’s needs are individually assessed and the patient’s length of stay is evaluated regularly by our interdisciplinary team. Room and board is not covered under the hospice Medicare benefit however, all or part of this cost may be covered by private insurers. Our hospice social worker is ready to assist you in helping to make the appropriate financial assessments.

Amenities

The Thomas P. Perciak Family Residential Hospice offers patients and families a variety of amenities including:

  • Each room is a private room that includes a private bath, patio and accommodations for families to stay overnight.
  • Pets are welcome to visit.
  • Food service is available for patient and guests.
  • Our facility features include a:
    • Kitchen
    • Two family rooms
    • Meditation Room
    • Children’s play area
    • Outdoor fountain garden area
    • Labyrinth for spiritual reflection
    • Community Room with fireplace, piano and television

Visitors

Visitors may visit 24 hours a day as long as that meets the patient’s wishes. Visitors must enter through the front vestibule at all times. The doors are locked at 8:00 p.m., so please ring the doorbell when arriving after that hour. Children are welcomed visitors at the residential center and our community room has age-appropriate toys, games and reading materials. For the safety of all children, we ask that an adult accompany them at all times.

Driving Distance

The Thomas P. Perciak Residential Family Center is 20 minutes from the Cleveland Hopkins International Airport; 30 minutes from the heart of downtown Cleveland; 10 minutes from the Ohio Turnpike I-80; 15 minutes from I-71 and 25 minutes from the intersection of I-480 and I-71.

Driving Directions

Coming south on I-71: Take the 82 west Exit. Make an immediate left onto Howe Road. At the Drake Road intersection, turn right. Turn left at the Strongsville Medical Center/Hospice Services sign. We are the first building on the left with a u-shaped driveway, 18659 Drake Road.
