The Southwest General Medical Group, Inc. includes both primary care and specialty care physicians who are committed to providing patients with high-quality medical care. The group features board-certified specialists and skilled physicians with years of experience in caring for patients. Together, they work to raise the bar for excellence in health care service for our community. Our physicians work closely with patients to ensure optimum health and wellness by focusing on prevention and early detection of disease as well as management of chronic illnesses.

Telehealth Patient Visits

As the COVID-19 pandemic unfolds, our communities and our health system are faced with unprecedented challenges. At Southwest General, the health and safety of our patients and caregivers remains a top priority, and we are working hard each day to continue providing the quality care you deserve with the compassion that has been our hallmark since our founding 100 years ago.

One way our Southwest General Medical Group (SGMG) providers are meeting patient needs during the COVID-19 pandemic is by offering telehealth patient visits – both by video (online) and by telephone. Learn more.

Hospital Care and Outpatient Services

Southwest General Medical Group, Inc. physicians are affiliated with Southwest General and offer hospital care and outpatient services at the hospital’s Main Campus or at its conveniently located facilities in Brunswick, Middleburg Heights and Strongsville. Our physicians have access to the full resources of Southwest General. This includes access to the hospital’s electronic medical record system, which helps to facilitate care and enhance communication and efficiency.

Managing Your Health

As a Southwest General Medical Group patient, you will be asked to set up a Southwest General HealtheLife account to help manage your health and communicate with your physician. Southwest General’s HealtheLife is a patient portal that gives you secure, online access to portions of your hospital-based electronic medical record. It also provides access to a wide variety of resources to help you more easily manage your health and wellness.

Once you have a Southwest General HealtheLife account, you can enjoy accessing your health information in comfort and privacy around the clock from anywhere you have internet access. You also can request appointments, message your physician and more. There is no fee to set up or use your account. Be sure to ask your caregiver for information to set up your account at your next visit.

Phone Calls

Phone calls are accepted during office hours. If you need to speak with a physician after hours, please leave a message with the answering service to contact the physician on call. The doctor will return your call as soon as possible. If you have an emergency, please go the closest Emergency Room or call 911!

Prescriptions and Refills

You may send us a prescription renewal request online using your HealtheLife account. If you do not have a HealtheLife account, please call during office hours to renew your prescription. Requests are handled within 24 business hours. You will need to provide the following information for prescription refill requests:

  • Medication name
  • Strength (dosage) of medication
  • How often medication is to be taken
  • Quantity needed (e.g., 30 day or 90 day supply)
  • Pharmacy name, street and phone number

Test Results

Results of lab, radiology and pathology tests done at Southwest General will be posted to your HealtheLife account within 24 hours after they are completed and entered into your Southwest General electronic medical record. Results for testing done outside of Southwest General (i.e., at another facility) may not be posted to the HealtheLife portal.

For testing done outside of Southwest General, and for patients who do not have a HealtheLife account, we will contact you with test results once your physician has received and reviewed the results.

Additionally, we will contact any patient by phone if there are further directions/suggestions from your physician once he or she has reviewed your test results.

Insurance Coverage

Southwest General Medical Group, Inc. physicians are approved providers for many insurance plans. However, please be aware that it is your responsibility as a patient to contact your insurance company by calling the member services phone number to verify that the physician you are scheduling with is an approved provider on your insurance plan. Please bring your current insurance card and a photo ID with you to all appointments.

Referrals/Pre-certifications

If you need to see a specialist in the Southwest General Medical Group, your insurance company may require a referral. It is your responsibility as a patient to determine if your insurance requires a referral, to verify that the specialist is on your plan, and to obtain a referral before visiting the specialist. Please allow two to three business days for us to obtain authorization from your insurance company.

Billing/Payment Information

Co-pays are expected at the time of your visit, when checking in for your appointment. Remember to bring your insurance card with you to each visit for verification. If you are paying for medical bills yourself and insurance is not involved, you will be asked for payment in full at the time of service. Please notify us of home address, phone number and/or insurance changes. This will allow for proper contact and billing.
