What Type of Visit Do I Need?
To help meet patient needs, Southwest General Medical Group (SGMG) providers
offer several safe and convenient appointment options—including
telehealth (virtual) visits. Options include:

In-person Visits
An in-person office visit may be scheduled for any non-emergency care that
requires in-person assessment, treatment or procedures. Both current and
new patients are invited to schedule an in-person appointment in one of
the following ways:
- Call the provider’s office directly
- Use your Southwest General HealtheLife account to send a message requesting
an appointment. Someone from the office will then contact you about your
appointment.

Scheduled Telehealth/Virtual Visits
If you do not need in-person care and prefer to visit with your provider
from the convenience of home, you can schedule a telehealth (virtual)
visit. Telehealth visits may be scheduled by current SGMG patients.

Important Appointment Reminders

What to bring to your appointment - You should bring your insurance card and a photo ID with you to all appointments.

Same day appointments – Some same-day appointments may be available with your provider.

Cancellations -To cancel or re-schedule an appointment, please be sure to call your provider’s
office or send a message via your Southwest General HealtheLife account
at least 24 hours in advance of your appointment. This will allow us use
the time to accommodate other patients.
